Transition Program for Young Adults
Joslin Clinic offers a transition clinic program for young adults with diabetes who are either newly diagnosed or transitioning from Pediatric to Adult Care.
Patients involved in Joslin's Transition Care Program have their care coordinated by a team of endocrinologists, diabetes nurses, nutritionists, exercise physiologists, and psychologists who specialize in young adults with diabetes.
Why Joslin?
• Approach. At Joslin, we offer 1:1 visits and classes with highly-qualified nurse educators, exercise physiologists and dieticians.
• Expertise. Our physicians specialize in young adults and have deep expertise in exercise physiology and extensive experience with technologies, including insulin pumps and Continuous Glucose Monitors.
• Experience. We are accustomed to working with a diverse set of patients with variable glucose control, as well as patients who may be dealing with the emotional demands of living with diabetes. We help manage diabetes on a day-to-day basis by offering expert medical advice and treatment in a non-judgmental setting.
• Convenience. We offer flexibility in scheduling and the ability to see multiple clinicians in one visit.

Experiences of health care transition voiced by young adults with type 1 diabetes: a qualitative study
This qualitative study aimed to explore the experience of transition from pediatric to adult diabetes care reported by post transition emerging adults with type 1 diabetes (T1D), with a focus on preparation for the actual transfer in care.
